The AdTran 622v is the optical network terminal (ONT) we use for our 10Gb fiber service. Essentially, it functions as a modem by providing 10 Gigabit Ethernet as well as phone jacks. The ONT is generally located indoors, and must be adjacent to a power outlet. 
The LED indicators on the AdTran 622v run along the front of the unit. Refer to the table below for an explanation of indicator behavior.

The ports on the back of the unit are:

1: 10 Gbps XGS-PON SC/APC fiber connector (WAN)
2: Two POTS (Phone) Ports. Use POTS1 for services with 1 phone line
3: 10 Gbps Ethernet Port
4: 1 Gbps Ethernet Port (This port is disabled)
5: Reset Button
6: Power Connection for the Supplied AC to DC Power Converter (12 V)
7: On/Off Button
